Well i have seen few tabs for this wonderful song. but here is my attempt, which i think 
pretty close.

LEAD OUT CHORDS:   C - C/B - Am  - D (D7) - 
F (Fmaj7)

Note: As always listen to the song for timing. in the lead out it sounds much better to 
the normal chords with the chords in brackets for example a quick change from F to Fmaj7 
back into the C sounds much better then the standard F to C change.
