Not sure if this is the exact way Mr. Hansard plays it (though I've seen him do it live
and he does capo it on the 4th fret and there are definitely some variations of all the
chords used here) but it's pretty darn close.
Sorry the spacing of the chords to match where they fall with the lyrics isn't accurate,
you'll have to listen along with the song to figure that out (if you don't already know
it well enough to just do it).  The way the tab editor is makes it all weird when you 
space stuff out.
