This song is from Sons of Anarchy S06E13, "A Mother's Work". I tried to make
the placement of the chords as close to when they're played in the lyrics as
possible. I didn't include the strumming pattern but I think he makes it
audible enough to figure out on the recording. Also, he likes to hammer on
the second lowest note of the chord being played during the first strum of
that chord. If you listen closely you'll hear what I'm talking about. 
For example, if you're playing Am, you'll lead in with:
